"Fractional CTO" has become a catch-all label. For a founder-led healthcare or regulated software team, the more useful question is not what to call the role, but what work actually needs an owner right now.

A fractional CTO, in the strict sense, is an executive layer: board-facing technology strategy, fundraising diligence support, vendor and budget authority, and the long-horizon technology bets. It is most valuable when the company is making company-level decisions and needs a credible technical voice in the room.

A Fractional Head of Engineering owns the engineering organization's ability to ship: architecture, sequencing, standards, hiring, code review, and the operating cadence that turns intent into delivered, safe software. It is most valuable when the company already knows roughly what it wants to build and the risk is in building it correctly.

The signal that you need the Head of Engineering role

Most teams that reach out think they have a strategy problem. More often they have an execution-leadership gap.

The tells are concrete:

  • Decisions get made, then quietly re-litigated, because no one owns the call.
